Payment & Insurance 

If you are a self pay patient, for your convenience we accept VISA, Mastercard, bank checks, money orders or cash. Please bring payment with you the morning of your procedure.

 

The Surgical Institute participates with Horizon BCBS, United Healthcare, Oxford, Cigna, QualCare, Aetna and Medicare. For further inquiries regarding your insurance provider, please contact our office. 

 

Financial Arrangements

The fee for use of the Surgical Institute includes routine equipment, supplies and drugs, operating and recovery room services. The fee does not include the professional services of your surgeon, anesthesiologist, radiology, pathology or implants. The Surgical Institute will bill your insurance company as a courtesy, however, the balance due is the patient’s responsibility.
